Request a Transcript

The Registrar's Office is the place to contact for information on how to resolve problems involving registration, schedule changes, adding or dropping classes, withdrawing from classes, auditing classes, grades, transcripts, graduation requirements and commencement. Degree verification requests may be made to the National Student Clearinghouse at www.nslc.org

Requesting a Transcript
Transcript requests should be directed to the Office of the Registrar and must be signed by the student or former student in compliance with Federal Law.

All requests must include:
  • Current name
  • Name when in attendance
  • Current address
  • Daytime telephone number
  • Student ID or Social Security Number
  • Written Signature
  • Destination Address

Cost: $6.00 per transcript request (Allow 3-5 business days for processing).

$10.00 per transcript request (Issued immediately - We do not process transcripts until payment is received.)

If you are sending your transcript to a large organization, it is advisable to include a department name or an individual's name at the organization that should receive your transcript.

We can also send transcripts by fax although many organizations will not recognize a faxed transcript as official.

You may print the transcript request form and send it to us via mail or fax. To request that a Transcript Release Form be sent to you via fax or mail, click here. We cannot accept transcript requests by e-mail or by phone because of the signature requirement.
